The New Version of Form I-604 (Determination on Child for Adoption)

Greetings from the Adoption Unit, American Consulate in Guangzhou! The ACIVU respectfully requests all adoption agencies to pass the following notice to adopting parents and adoption facilitators:
USCIS has begun using the new version of Form I-604, Determination on Child for Adoption, revised October 30, 2006 (previously referred to as "Request for and Report on Overseas Orphan Investigation"). Effective immediately, the U.S Consulate will begin using the October 30, 2006 version. We will start checking the new form on February 26, 2007.
Please assist your adoptive families to complete No.1, No,4, No.9 and No.14 of the attached new form. Please also help the U.S. Consulate to complete the last part of page 4 (see below). We will print the new form and send it to the families via mail in the near future.
