Important Notice: Electronic Applications Now Required For All Passport Services
Starting May 15 2009, the Guangzhou American Citizen Services Section will require use of electronic application forms for all passport services, including passport renewals and adding visa pages. After filling out your application on-line, print and bring your application to your appointment. You may fill out the required forms using the Passport Wizard at the following site: https://pptform.state.gov/?Submit=COMPLETE+FORM+DS-4085+ONLINE.
Failure to fill out applications on-line, instead using handwritten applications, will delay processing of your application.
Click here to make an appointment. All passport applicants, including children under age 16, must apply in person. Both parents need to provide unequivocal consent to passport issuance by signing the application form in front of the Consular Officer. If one parent is unable to attend the interview, he/ she must submit a notarized letter of consent, in English, that authorizes the other parent to apply for a passport for the child. If neither parent is able to attend the interview, each parent must submit a notarized letter of consent, in English, or submit form DS-3053 (Statement of Consent or Special Circumstances: Issuance of a U.S. Passport to a Minor under Age 16) that authorizes the child’s legal guardian to apply for a passport for the child. A copy of the parents’ identification must accompany the notarized consent letter or DS-3053.
Passport Renewal for Children (under 16, starting February 1, 2008):
Your child’s previous passport
Your child’s birth certificate/Consular Report of Birth Abroad (Form FS-240) /adoption decree. The documents should be either original or a certified copy.

Parents' or guardians' passports (If one parent is absent, the other should bring a notarized statement or notarized
DS-3053 authorizing the other parent to apply for the child’s passport. The absent parent should also provide a copy of his/her passport or ID. Alternatively, the parent applying for the passport may also submit proof of sole custody.)
If the child’s last passport was issued in the United States, a copy of the U.S. or Chinese passport or travel document the child used to enter China.
If the parents have authorized a legal guardian to apply for a passport on behalf of their child, the legal guardian must submit a notarized power of attorney in English that specifically authorizes him/ her to apply for the passport on behalf of the child. Both parents must sign the power of attorney and provide proof of their identification.
