Transportation Letter Application for Expired Green Card

  1. Valid Passport
  2. Permanent Resident Card copy if any
  3. Driver License or any ID card
  4. 3 passport color photos with white background. (Please see Photo Standards requirement on website uscis.gov for more information)
  5. Air ticket or boarding pass used to leave the U.S.
  6. Notarial Sworn Statement, with certified translation, regarding how, when received Permanent Resident Card, where resided in the U.S., when departed the U.S. (date/place), when arrived in China (date/place), and how you departed the U.S. with an expired Permanent Resident Card or why you did not return to the U.S. prior to date of expiration of the Permanent Resident Card.
