New Appointment Request Form

In order to improve our appointment processing, Consulate Guangzhou’s Adoptions Unit is implementing a new appointment request form. Beginning Thursday, August 27, 2009, all agencies need to use the new appointment request form.  Appointment requests should have all fields complete, or the appointment request will not be processed.  Any appointment requests made without the use of this form, or with incomplete information, will not be processed.  If an appointment request has already been made prior to Thursday, August 27, 2009, there is no need to “resend” the request using this new form.
