REQUIREMENTS FOR PASSPORT RENEWAL/ADDITIONAL PAGES

All application forms must be completed and sent to the American Citizen Services section of the U.S. Consulate General Guangzhou via fax or email prior to the date of the Outreach.  All application forms must be accompanied by a copy of the first page (biodata page) of your current passport.  We do not advise mailing your documents to us, as we cannot guarantee timely receipt of mail. 

American Citizen Services must receive your completed application form(s) and copy of your passport biodata page before 12:00 noon, Wednesday, April 6, 2011, or we will NOT be able to renew or add pages to your passport.  Please include your contact number and email address on your application. We will notify you by phone or email that we have received your application form and passport biodata page.

FOR ADDITIONAL PAGES IN YOUR CURRENT U.S. PASSPORT

Fax or email completed Form DS-4085 and copy of the first page of your passport to American Citizen Services.  The original, signed form along with the passport must then be brought to the Outreach session. 

U.S. passport holders 16 years of age and older may authorize a third party to bring the original application and passport to the Outreach.  A signed authorization letter with the name and identification number of the person submitting the application is required.

For minor U.S. passport holders under the age of 16, an original or certified copy of their birth certificate must be presented.  At least one parent (or authorized guardian) will be required to appear and submit the application form and passport at the Outreach. The child is not required to appear for this service.

The U.S. Department of State now charges a fee of USD $82.00 to add additional pages to passports.  Payment can be made in U.S. dollars or in Chinese Renminbi, which will be exchanged at the current daily rate in effect on the day of the Outreach as posted by the U.S. Department of State.   As 3/10/2011, USD $1.00 = RMB ¥6.70.  Cash only payment will be accepted and exact change will be appreciated.

Note:  If your passport has already had additional pages added two or more times, you will need to apply for a new passport.

FOR ALL PASSPORT APPLICATIONS

Fax or email completed Form DS-11 or Form DS-82, as noted below, and a copy of the first page of your passport (biodata page) to American Citizen Services.  The original, signed form along with the passport must then be brought to the Outreach session.

 1.    Completed passport application Form DS-11 for all minors under 16 years of age and all first-time adult passport applicants;   OR,

       Completed passport application Form DS-82,  for adults wishing to renew an adult passport.

2.    Your current U.S. passport.

3.    Two (2) identical, recent color passport photos (2” x 2” = 5 x 5 cm).  They must be full face, front view with a plain white or off-white background.  

4.    Original or certified copy of Birth Certificate for minors under 16 years of age.

5.    Pictures showing child’s facial development from issue date of expiring passport to within six months of time of application, for minors under 16 years of age.

6.    U.S. adult passport holders may authorize someone else to submit their original application and passport to the Outreach.  A signed authorization letter with the name and identification number of the person submitting the application is required.

7.     Required fees total US $110.00 for adults, and US $105.00 for minors under 16 years of age.  Payment can be made in U.S. dollars or in Chinese Renminbi, which will be exchanged at the current daily rate in effect on the day of the Outreach as posted by the U.S. Department of State.   As of 3/10/2011, USD $1.00 = RMB ¥6.70.  Cash only payment will be accepted and exact change will be appreciated.  (U.S. Department of State, current fee information).

Please be advised that U.S. law requires both parents of passport applicants under the age of 16 to appear for the passport interview. If only one parent is present, the other parent’s notarized Form DS-3053 or notarized written statement of consent (including the child's name and date of birth, as well as a copy of the parent's identification information) authorizing passport issuance is required.   Should one parent have sole legal authority to apply for a minor passport, supporting evidence must be presented at the outreach session.  A third-party in loco parentis applying on behalf of a minor under the age of 16 must submit a notarized written statement or affidavit from both parents or the parent having sole custody of the child authorizing passport issuance.

Please note that all passport applications must be forwarded to a domestic U.S. passport facility for issuance.   Current processing time for new passports is approximately two weeks.  Applicants, or persons properly authorized by applicants, must come to the U.S. Consulate to pick up their passports at which time their old passports will be cancelled and returned to the applicant for retention.

NOTARIAL SERVICES

Notarial services for U.S. citizens and non-U.S. citizens (for non-U.S. citizens, notarial services are only provided for documents to be used in the U.S.) require proper identification (see
acceptable forms of identification.) 

To better serve you during Outreach hours, should you require notarial services, please email us your name, local phone number and type of document to be notarized.  We will notify you by phone or email to confirm your request.

The U.S. Department of State now charges a fee of USD $50.00 for each notarial signature/seal.  Payment can be made in U.S. dollars or in Chinese Renminbi, which will be exchanged at the current daily rate in effect on the day of the Outreach as posted by the U.S. Department of State.   As of 3/10/2011, USD $1.00 = RMB ¥6.70.  Cash only payment will be accepted and exact change will be appreciated.
