Transportation Letter Application for Losing Green Card

  1. Valid Passport
  2. Police Report
  3. Permanent Resident Card copy if any
  4. Driver License or any ID card
  5. 5 passport color photos with white background. (Please see Photo Standards requirement on website uscis.gov for more information)
  6. Submit I-90(Permanent Card Replacement Application). This application can be obtained on the internet website uscis.gov
  7. Fee for the I-90(185 USD)
  8. Notarized Sworn Statement about the loss of the Permanent Resident Card (issued by the Notary Authority in China )In the statement, you have to provide the following details: Your Full Name, Date of Birth and Alien number, your address and employment location in the U.S., date of departure from the U.S. and date of arrival in China., place where you lost the green card, etc.
  9. Air ticket or boarding pass used to leave the U.S.
