Visa Application Form

Please be careful when seeking help from agencies, brokers and student consultants in completing your application.  Agents who encourage you to misrepresent yourself are not to be trusted and anyone who offers to sell you documents is acting illegally.  Lying in your interview and using fake documents can make you ineligible for a visa.

Online Electronic DS-156 All Non-immigrant visa applicants must submit Electronic Visa Applications Form (EVAF) DS -156 for their visa applications. The online DS-156 form contains a bar code, which will allow us to electronically transfer data. Once the application form is completed online, it must be printed, and then submitted to the Consular Section at the time of the visa interview. Please submit the original printed form, including the last page, which contains the bar code-photocopies or faxes of the printed form are not acceptable. In addition, please do not fold the printed application form. Click here to download the online DS-156 from the State Department website.

After the form is printed out, Chinese nationals must also fill out Items 6, 7, 8, 9, 15, 18, 20 manually in Chinese and must provide the telegraph code for their Chinese surnames and given names while filling out Items 6 & 7, which can be found at most Chinese post offices.

For now, DS-157 and DS-158 application forms are still non-electronic and cannot be filled out online. They can be downloaded from this page and you can type your answers into the forms and print them out.

DS-157 Supplemental Nonimmigrant Visa Application
All applicants 16 years of age and older must complete the DS-157 application form, which is one page. Click here to download the DS-157 form, type your answers in the space provided below each item and print out the completed form. You don’t need to fill out a Chinese DS-157, but if you need its Chinese translation to help understand the English form, click here. (Note: When filling out Item 3 “Full Name (In Native Alphabet)”, Chinese applicants should write down their full name in Chinese characters. Chinese applicants must also fill out items 5, 6, 7, 12 & 17 in Chinese characters.)

DS-158 Contact Information and Work History for Nonimmigrant Visa Applicant
All Chinese applicants applying for F (Student), J (Exchange) and M (Non-academic Student) visas, and their dependents (F-2, J-2, M-2), must complete the DS-158 application form, which is two pages. Click here to download the DS-158 form, type your answers in the space provided below each item and print out the completed form. You don’t need to fill out a Chinese DS-158, but if you need its Chinese translation to help understand the English form, click here.
